Advanced Clinical Practitioner – HMP Holme House

Spectrum have an exciting opportunity to join our team at HMP Holme House working as an Advanced Clinical Practitioner supporting our primary care team.

Main duties of the job

The role entails running clinics, responding to urgent queries, oversight of complex cases and supporting nursing staff managing long term conditions. The role also leads on training and education for junior staff.

Job responsibilities

You will work alongside the Head of Healthcare to provide clinical leadership to the team.

Advanced Clinical Practitioner Qualification

  • Professional regulated core qualification with the NMC/HCPC/GPhC
  • Post graduate qualification in NMP and Advanced Health Assessment
  • Nationally recognised mentorship/ teaching qualification
  • Qualifications in Project Management and/or Quality improvement
  • Mental health/Learning Disability post graduate qualification
  • ECG interpretation module
  • RCGP Certificate in the Management of Drug Misuse Level 1 and 2

Knowledge

  • Extensive experience in a management/clinical/practitioner leadership role.
  • High level of clinical expertise in working with people with complex care needs
  • Extensive experience of working in primary care with a patient centred approach.
  • Experience of developing clinical pathways and intervention plans
  • Experience in teaching/ clinically supervising junior staff.
  • Experience of leading/managing a multidisciplinary team. Experience of leading on a series of audits, action plans and projects simultaneously.
  • Experience in assessing, formulating and managing risk for people with severe and enduring physical health conditions
  • Experience in clinically leading staff groups.
  • Experience in developing and adapting systems and processes to meet the needs of the individual and the service
  • Experience of clinically leading a team.
  • Experience of working in a forensic health or prison setting
  • Experience of service development and transformation
  • Experience of working in a physical health setting.
  • Experience of working within substance misuse.
