Advanced Practitioner – Adults

About the Role

Wirral Council is seeking an experienced Advanced Practitioner to join its busy Wallasey Adult Social Care Team. The role involves working alongside another Advanced Practitioner to support complex and high‑risk cases and to shape best practice across the service.

Responsibilities

• Manage a complex caseload involving safeguarding, high‑risk situations, and Court of Protection work
• Support practitioners with challenging cases and provide professional leadership and mentoring
• Lead safeguarding processes, including chairing protection meetings
• Ensure person‑centred assessments, care planning, and effective risk management
• Work with partners and safeguarding teams to improve practice and outcomes
• Contribute to service development, innovation, and policy improvements
• Quality assure reports, assessments, and court documentation
