About the Role

We are seeking an enthusiastic and patient-focused Clinical Pharmacist to join our friendly multidisciplinary team. This is an exciting opportunity to deliver high-quality, patient-centred pharmaceutical care and support the safe and effective use of medicines within our GP practice. The post holder will work within their professional boundaries, supporting and collaborating with GPs, nurses, pharmacy technicians and other clinicians. Working in a patient-facing role, they will provide clinical pharmacy services, medicines optimisation and prescribing support to improve patient outcomes. The post holder will support practice staff with prescription and medication queries and safer prescribing processes. They will provide expert medicines advice while addressing the health and social care needs of registered patients. The successful candidate will work alongside our current Clinical Pharmacist within a supportive and collaborative environment, with opportunities to share expertise, develop clinical skills and contribute to service improvement.

Practice Overview

The Market Cross Surgery in Mildenhall, Suffolk is an Outstanding‑rated CQC practice caring for approximately 12,700 patients. It is a single‑site dispensing practice and a GP Registrar training practice, using SystmOne as the clinical system and part of a proactive Primary Care Network.

Key Responsibilities

  • Deliver high‑quality, patient‑centred pharmaceutical care and support the safe and effective use of medicines within the GP practice.
  • Work within professional boundaries, supporting and collaborating with GPs, nurses, pharmacy technicians and other clinicians.
  • Provide clinical pharmacy services, medicines optimisation, and prescribing support to improve patient outcomes.
  • Support practice staff with prescription and medication queries and safer prescribing processes.
  • Provide expert medicines advice while addressing the health and social care needs of registered patients.
  • Work alongside the current Clinical Pharmacist within a supportive and collaborative environment, sharing expertise, developing clinical skills and contributing to service improvement.
  • Contribute to medicines optimisation, quality improvement, QOF and Enhanced Services where appropriate to improve patient outcomes and support efficient service delivery.
  • Patient‑facing long‑term condition (LTC) reviews – identify patients with conditions such as COPD and asthma, review ongoing need for medicines, monitoring needs, and support patients to take medicines effectively; make recommendations to senior pharmacists or GPs.
  • Patient‑facing clinical medication reviews – conduct structured medication reviews with patients and produce recommendations for nurses and/or GPs on prescribing and monitoring.
  • Patient‑facing domiciliary clinical medication reviews – conduct reviews with patients at home, producing recommendations for senior pharmacists, nurses and/or GPs.
  • Management of common, minor or self‑limiting ailments – manage the caseload of patients with such conditions, signpost to community pharmacy, and refer to GPs or other health‑care professionals where appropriate.
  • Patient‑facing medicines support – run patient clinics and telephone help lines for questions and concerns, and provide follow‑ups to monitor the effect of any changes.
  • Medicine information to practice staff and patients – answer enquiries from GPs, practice staff, other health‑care teams and patients, suggesting and recommending solutions.
  • Unplanned hospital admissions – conduct audits of medicines associated with admissions and readmissions, implementing changes to reduce prescribing of high‑risk medicines.
  • Medication reconciliation at discharge – reconcile medicines following hospital discharge and other transfers of care, identify and rectify unexplained changes, and work with patients and community pharmacists to ensure continuity of medicines supply.
  • Repeat prescribing – review practice repeat prescribing policies, manage the repeat prescribing re‑authorisation process, review medicines reaching review dates and flag those requiring review; ensure appropriate monitoring tests are in place.
  • Risk stratification – identify cohorts of patients at high risk of harm from medicines through pre‑prepared computer searches, including patient‑related, medicine‑related or combined risks.
  • Service development – contribute pharmaceutical advice for new services with medical components, and analyse, interpret and present medicines data to support decision‑making.
  • Medicines quality improvements – undertake clinical audits of prescribing, provide feedback and implement changes in conjunction with the practice team.
  • Medicines safety – respond to MHRA alerts, product withdrawals and other national guidance; implement local and national guidelines and formulary recommendations.
  • Audit compliance – audit practice compliance against NICE technology assessment guidance and ensure compliance with CQC standards where medicines are involved.
  • Public‑health support – support public‑health campaigns and provide specialist knowledge on all public‑health programmes available to the public.
  • Collaborative working relationships – recognise other colleagues’ roles, maintain effective communication, and foster strong links with all services across the locality, including community and hospital pharmacy teams.

Qualifications

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council (GPhC).
  • Experience of working as a pharmacist within a clinical setting.
  • Understanding of medicines optimisation and safe prescribing principles.
  • Awareness of common acute and long‑term conditions managed within primary care.
  • Experience providing patient‑centred consultations and medicines advice.
  • Ability to recognise clinical priorities, identify risks and escalates appropriately.
  • Understanding of professional, legal, and ethical responsibilities.
  • Leadership and management skills – knowledge of pharmacy role in governance and ability to implement it appropriately.
  • Knowledge of public‑health programmes and ability to engage with patient participation groups.
  • Independent Prescriber qualification – completion of the Primary Care Pharmacy Education Pathway (PCPEP) or equivalent postgraduate clinical pharmacy training; relevant postgraduate clinical pharmacy qualifications; experience undertaking Structured Medication Reviews (SMRs); familiarity with clinical systems such as SystmOne; experience supporting prescribing audits, quality improvement projects and medicines safety initiatives.
