Job Description:

We are looking for two highly motivated and enthusiastic individuals to join our expanding Pharmacy and Medicines Optimisation team at HPFT, in newly created posts supporting community mental health services. These roles provide an exciting opportunity to help shape and develop mental health clinical pharmacy services across our community teams in Hertfordshire.

Main Duties of the Job

  • Clinical and professional practice: Provide a specialist clinical pharmacy service in the community setting, ensuring safe, effective and evidence-based use of medicines; conduct comprehensive medication reviews; support high‑risk medicines optimisation; promote adherence while working collaboratively within multidisciplinary teams.
  • Safety: Reduce medication‑related risks by supporting safe practice through identifying, reporting and investigating incidents.
  • Leadership: Plan and manage own workload, support pharmacy service modernisation, and contribute to the development of medicines optimisation initiatives in the community.
  • Education and training: Provide training and education that meet statutory, professional and regulatory requirements for pharmacists and technicians.
  • Participation: Late duty, weekend, bank holiday and on‑call rotas may be required to support equitable seven‑day services.

Qualifications and Experience

  • Degree in Pharmacy (BPharm, MPharm, BSc Pharmacy) and registration with the General Pharmaceutical Council (GPhC).
  • Postgraduate clinical pharmacy qualification (or equivalent experience) with evidence of ongoing CPD.
  • Post‑registration mental health clinical experience, including multidisciplinary working, clinical pharmacy provision and audit completion.
  • Experience in self and staff development and change management.
  • Strong organisational, planning and prioritisation skills, with experience of delivering training to healthcare professionals.
  • Excellent written and verbal communication skills; sound knowledge of mental health medicines.
  • Demonstrated CPD and, where possible, registration with the Royal College of Pharmacy (RCPharm).
  • Postgraduate certificate or diploma in Mental Health Pharmacy or equivalent experience.
  • Postgraduate diploma in Advanced Clinical Practice Management or equivalent leadership qualification.
  • Independent prescriber status or working towards it.
  • Experience in working in a multidisciplinary team, completing audits, managing workload and service development within an NHS organisation.
  • Management skills including recruitment, supervision and appraisal.
  • Experience writing and reviewing policies, guidelines and SOPs.
  • Ability to influence other professionals and make judgements involving complex facts.
  • Car driver licence and access to a vehicle for business use.
