Position Summary
This role allows nurses to deliver crucial mental health support and interventions directly within the community, positively influencing patients’ lives. Key Responsibilities
- Identify appropriate care pathways and ensure the least restrictive option for patient wellbeing.
- Signpost patients to appropriate care providers including primary and tertiary care.
- Raise concerns to senior staff for support and management.
- Complete documentation and care planning for patients.
- Present cases to the Multi‑Disciplinary Team for discussion of care pathway.
- Liaise with colleagues across nursing, occupational therapy, medics, psychology, and admin.
- Perform any additional duties as required.
Required Qualifications and Experience
- A minimum of 6 months paid experience as a Community Psychiatric Nurse in a UK hospital within the last 3 years.
- Full UK driving licence and own vehicle.
- Compliance with NMC professional registration and up‑to‑date nursing practice.
- Depending on the Trust, variable additional certification and skills may be required.
- Flexible bank shifts across any of our client Trusts in England.
