Job Summary
Social Worker (level 2) – permanent contract – 37 hours per week. Salary £39,862 – £41,771 per annum (Grade J). The post is based at Priory House in King’s Lynn as part of the West Follow‑On Team. The role supports people following discharge from hospital, whether returning home or moving into care placements. The incumbent will manage their own caseload, undertake Care Act assessments, complete Mental Capacity assessments and lead Section 42 safeguarding enquiries.

Responsibilities
- Manage own caseload and work with confident decision‑making.
- Conduct Care Act assessments.
- Complete Mental Capacity assessments.
- Lead Section 42 safeguarding enquiries.
- Build partnership working and use a person‑centred approach.
- Support people to build on their own networks, relationships and local resources to achieve outcomes that matter most.
Qualifications
- Registered with Social Work England as a practising Social Worker.
- Level 2 Social Worker experience (Grade J and above).
