Clinical Pharmacist — Primary Care Impact in Leicestershire

DHU Healthcare in Leicestershire is seeking a Clinical Pharmacist to deliver expert medicines management across GP practices and community settings. You will conduct structured medication reviews, medicines reconciliation and support patients with complex polypharmacy.

Working in a multidisciplinary team, you will help manage long-term conditions and contribute to safe, patient-centred care across Primary Care networks, NHS 111 and out-of-hours services.
