Nightingale Home from Hospital Service (Casual Contract)

We are looking for a flexible and proactive individual to join our Nightingale Home from Hospital service, which provides free support to local older people discharged from hospital or who become unwell in the community, with a particular emphasis on those living alone and without family support.

• Experience in a care related field such as social care, health or therapeutic setting
• Good level of IT skills
• Access to a vehicle
