Job Description:

We are looking for an Advanced Care Practitioner to join our supportive and experienced team.

As an Advanced Care Practitioner, you will work across Copeland as part of a diverse team of clinicians to provide access for patients – increasing the capacity to meet the demand for appointments. The Advanced Care Practitioner will see and treat acute patients and support them in the management of ongoing and chronic conditions.

Clinical

  • Making professionally autonomous decisions
  • Managing patients with undifferentiated and undiagnosed problems
  • Provide comprehensive healthcare sensitive to patients needs
  • Conducting physical assessments to diagnose patients conditions
  • Direct and participate in provision of continuing care
  • Managing and participating in Chronic disease management
  • Supporting and developing work towards agreed GMS targets
  • Any other duties as may be required by the practice in providing quality health care to patients
  • Provide comprehensive healthcare sensitive to patient needs

Performance and Quality

  • Ensure that accurate performance data is recorded
  • Ensuring patient records are maintained accurately
  • Ensure that significant events are recorded and actions implemented
  • Where appropriate share and or elevate complaints and significant events
  • Liaison with other service providers to ensure effective care is delivered
  • Maintain clinical knowledge and skills and identify training needs
  • Comply with any regulatory, professional or registration requirements relevant to the role

Communication

  • Develop and maintain effective professional relationships with other members of the clinical, administrative and management teams of NCPC

NCPC (North Cumbria Primary Care) was established in August 2019 in response to an increasingly fragile general practice provision within North Cumbria. NCPC’s vision is to bring practices together to develop a sustainable, fully integrated primary care – built locally and delivered together.

The Copeland NCPC area consists of 3 major practices, Fellview Health Care, Lowther Medical Centre and Seascale Health Centre as well as a shared PCN (Primary Care Network) which delivers services to patients across all 3 practices.

We are a not-for-profit company with all money we receive in funding and other business activities going towards supporting the organisation, our staff, and our services.

Advanced Care Practitioner

Role Purpose

The Care Practitioner will work as part of a team of diverse clinicians to provide access for patients and increase capacity and meet patient demand for appointments. The Care Practitioner will see and treat acute patients and support the management of ongoing and chronic conditions.

As part of the wider Healthcare Team there is the opportunity for practitioners to bring or develop particular specialities which support the health needs of our population. The post holder will work with the clinical and management team to ensure that staff and patients remain safe and services are delivered in adherence to agreed standards, policies and procedures.

Duties and Responsibilities

Clinical

  • Making professionally autonomous decisions
  • Managing patients with undifferentiated and undiagnosed problems.
  • Provide comprehensive healthcare sensitive to patients needs, including assessment, diagnosing, prescribing, and referrals where appropriate. Complementing the roles of GPs and the multidisciplinary team.
  • Conducting physical assessments to diagnose patients conditions and provide appropriate self-care advice, prescribing treatment, or referral using evidence based guidelines.
  • Direct and participate in provision of continuing care, health maintenance, screening and health promotion to patients as part of patient-centred multidisciplinary team.
  • Managing and participating in Chronic disease management
  • Supporting and developing work towards agreed GMS targets.
  • Any other duties as may be required by the practice in providing quality health care to patients.
  • Provide comprehensive healthcare sensitive to patient needs, including assessment, diagnosing, prescribing, and referrals where appropriate. Complementing the roles of GPs and the multidisciplinary team.

Person Specification

  • The applicant must hold the relevant qualifications as well as current NMC membership.
