VONNE (Voluntary Organisations’ Network North East) in Sunderland seeks a dedicated Social Prescribing Service professional to deliver a locality-based health and wellbeing programme for older people. The role focuses on prevention, independence, and strengthening community support.

We require NVQ Level 3 in Health & Social Care (preferred), a valid driving license with access to your own vehicle, and a DBS Enhanced Disclosure. This is a full-time permanent post with a fixed salary.
