HUC is seeking a motivated WorkWell Coordinator in Welwyn Garden City. You will manage a diverse caseload, conduct holistic assessments, and co‑produce personalised WorkWell Plans to help individuals improve health, wellbeing and employment outcomes.
You will work closely with healthcare professionals, authorities and partner organisations, coordinating referrals and monitoring progress to empower people to overcome barriers and achieve sustainable work outcomes.
