Schedule: Full-time – U.S. business hours (EST)
Salary: $1,200 USD/month (depending on experience)
We are seeking a licensed nurse to support a U.S.-based endocrinology practice through its Principal Care Management (PCM) program.
PCM is a care coordination program designed for patients with a single chronic condition requiring ongoing management. In this role, you will work closely with the provider to ensure patients receive continuous support between visits, helping improve outcomes and adherence to treatment plans.
This is a clinical, patient-facing role, requiring strong judgment, communication skills, and the ability to actively monitor and guide patients throughout their care journey.
Key Responsibilities
- Maintain ongoing communication with patients enrolled in the PCM program
- Monitor patient conditions, including:
- Symptoms
- Medication adherence
- Progress and response to treatment
- Provide patient education regarding their condition and care plan
- Assist in the implementation and follow-up of care plans
- Coordinate services and ensure continuity of care
- Identify changes in patient status and escalate concerns to the provider when needed
- Ensure patients follow treatment recommendations and scheduled appointments
- Accurately document all patient interactions and updates in the system
Requirements
- Advanced English level (C1 or higher) – excellent spoken and written communication
- Proven experience working with U.S.-based medical offices
- Experience with chronic care management, patient follow-up, or care coordination
- Strong clinical judgment and ability to assess patient conditions remotely
- Experience providing patient education and support
- Highly organized with strong documentation skills
- Comfortable working independently in a remote environment
