Overview

An exciting opportunity has arisen with the Asthma Clinical Nurse Specialist Team.

Responsibilities

Duties will be to manage own defined caseload of patients within the agreed scope of practice and taking responsibility for continuity of care acting as an independent practitioner. This will include assessing patients often with highly complex or chronic conditions, providing specialist advice across professional boundaries, planning and implementation of care, running nurse‑led clinics and comprehensive follow‑up services, and ensuring a smooth transition for patients through the relevant pathway.

  • Manage a caseload of patients undertaking physical assessment and assume responsibility for requesting surveillance investigations relevant to the specialism, evaluating results and instigating any necessary action.
  • Maintain seamless lines of communication with patients, secondary and primary care, and oversee management of resources to promote effective patient care and achieve national standards.

All clinic work will be delivered under the indirect supervision of a named Consultant.
