We're looking for an enthusiastic dedicated nurse who is keen to work in our Frailty (SDEC) Same Day Emergency Care. You will be working in the Frailty Directorate, Medicine Division, at an acute hospital in a care of the elderly setting.
This service focuses on delivering rapid assessment and bespoke care to our elderly patients, providing treatments and interventions to prevent prolonged hospital stays and unnecessary admissions.
Responsibilities
- Triage patient referrals and execute timely interventions.
- Manage admissions and discharges, prioritise and delegate workload.
- Conduct audits related to nursing professional development and monitor standards of care.
- Act as an agent for change, fostering a flexible culture that responds to the demands and future development of the service.
- Ensure the privacy and dignity agenda of the unit.
- Work in an unpredictable environment, including violence and aggression and clinical emergencies.
- Operate during a 5‑day service Monday‑Friday and eventually enter a 7‑day rota as the service expands.
- Participate in continuous quality improvement and contribution to patient outcomes.
Qualifications
- Highly motivated registered nurse committed to delivering high standards of care.
- Excellent clinical and communication skills.
- Ability to work effectively and efficiently within team objectives.
- Experience or willingness to develop in a frailty nursing environment.
