We are recruiting a clinical pharmacist at Band 7 (Contract Permanent) for a 37.5‑hour week, including on‑call cover and late‑night duty. Main duties of the job

We are looking for a highly motivated individual with post‑registration training and clinical experience. The ideal candidate will have good clinical skills and be keen to support innovative changes.

The post holder will be responsible for delivering a high quality, clinically robust pharmacy service to wards and other clinical areas, supporting safety alert action plans, teaching medicines management to multidisciplinary staff, and working with senior pharmacists. Rostered on‑call and weekend cover are required.

Detailed job description and main responsibilities

  • Provide high quality pharmaceutical care to patients and carers by undertaking ward rounds and attending MDTs for surgical patients.
  • Work closely with medicines management technicians to ensure delivery of an integrated pharmacy service across the hospital.
  • Develop expertise in using the Trust EPMA system and help other users improve medication reconciliation, prescribing, administration and discharge planning.
  • Demonstrate and utilise clinical pharmacy knowledge through personal development.
  • Collaborate within the multidisciplinary team to deliver excellent patient care through specialist and consultant ward rounds, clinics and MDT meetings.
  • Act as a role‑model in clinical pharmacy and provide a consistently high level of service in line with Trust objectives and values.
  • Audit, monitor and develop medicines optimisation in line with pharmacy strategy aims and objectives.
  • Work as an autonomous pharmacist within appropriate clinical and professional guidelines in a managed environment, ensuring safe and effective medicine use.

Person specification

Qualifications

  • Registered Pharmacist with the GPHC.
  • BSc, BPharm, MPharm.
  • Successfully completed the PG Pharmacy Clinical Diploma or equivalent experience.
  • Independent Prescribing Qualification (or working towards).

Experience

  • Significant experience working in an acute NHS Trust.
  • Clinical experience and knowledge gained in a range of specialties.
  • Previous use of electronic patient record.

Knowledge

  • Sound clinical knowledge.
  • Evidence‑based practice.

Skills

  • Demonstrates problem‑solving skills.
  • Excellent written, verbal and presentation communication.
  • Able to work autonomously, independently and as part of a team.
  • Experience of data analysis.
  • Analytical ability for problem solving.
