Service Coordinator – Assisted Discharge Hours: 35 per week Monday to Friday 10am – 6pm or 9am-5pm

Contract: Fixed term contract until 31st July 2026

Driving Requirement: Full UK Manual Driving License Required (Mandatory) with 2 years driving experience. Business insurance on own car. As a Service Coordinator , you’ll be the vital link between hospital referrals and care at home. You’ll manage a team of staff and volunteers, ensuring services are delivered safely, efficiently, and with heart. What will a day in the life of a Service Coordinator involve? Acting as the main point of contact for hospital and community referrals. Coordinating and supporting a team of staff and volunteers to deliver services. Monitoring service delivery, maintaining accurate records, and ensuring quality standards are met. Attending hospital meetings, managing the service budget, and stepping in to support frontline delivery when needed. To be a successful Service Coordinator, what will you need? Confidence using IT systems and managing service records. A flexible approach, with a willingness to work evenings, be on-call, and respond to emergencies.
