Band 5 Staff Nurse – Virtual Ward
We have an exciting opportunity for a passionate and driven Band 5 Staff Nurse to join a dynamic healthcare team focused on delivering high‑quality, patient‑centred care within a Virtual Ward setting, based at Sheppey Community Hospital and covering the SWALE area.This role involves delivering and developing a new service that supports patients to recover from or manage their illness and enables them to stay healthy and well for longer.
Responsibilities
- Participate in assessment and care planning of patient care in conjunction with the Senior Sister/Team Sister on the virtual wards.
- Implement and evaluate care and treatment in agreement with the patient and as defined in the care plan, escalating any deviation or change in condition to the Senior Sister/Team Sister, Charge Nurse or GP as appropriate.
- Communicate effectively with patients, carers and all members of the multidisciplinary team.
- Prioritise daily work in line with clinical and patient need.
- Maintain accurate and legible patient records in line with professional and organisational requirements, including electronic patient record systems.
- Ensure evidence‑based care and best practice in all aspects of care delivery.
- Actively participate and contribute to team meetings and daily handover.
Qualifications
- Registered Nurse with the NMC (Band 5) or equivalent.
- Evidence of continuing professional development.
- Mentorship or teaching qualification within the first 2 years of post.
- Degree level education.
- Full UK driving licence and access to a vehicle for work purposes.
