We invite you to apply for the Band 5 Mental Health Practitioner position in the South Memory Assessment Service, Cambridge and Ely. We are looking for someone with a keen interest in dementia and older people’s care. This post is open to registered nurses with the right attributes to enhance the developing Memory Assessment Service.
Responsibilities
- Contribute to the assessment of memory functioning and participate in the treatment process including post-diagnostic support.
- Support service users and carers, offering information and signposting them to other agencies.
- Work closely with colleagues from the multi-disciplinary team, psychiatrists, nurses, psychologists and others.
- Provide the necessary support to ensure your development within your role.
- To work within the integrated multi-professional team and the Memory Assessment Service.
- To be accountable for contributing to assessing, interpreting, planning and implementing treatment and care to patients within professional guidelines, with the support of the Band 6 mental health practitioner.
- To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.
- Provide support and education to peers, new staff, non-registered staff and Students.
Requirements
- Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs.
