An exciting opportunity has become available for a Band 6 Community Mental Health Nurse who has the relevant qualifications and experience in Acute Mental health. The team is based in Newcastle Emlyn, Ceredigion.
The Community Mental Health Team is a multi-disciplinary team that undertakes various roles and duties including undertaking initial comprehensive assessment of people accessing the service. We offer care coordination under the Mental Health Measure (Wales) 2010 to support people through their recovery.
The Community Mental Health Team works closely with the wider mental health services within Hywel Dda, other statutory services and third sector services to ensure the people accessing mental health services are provided with the most appropriate care for their needs.
The ability to travel between sites in a timely manner is essential.
Main duties of the job
Our services work through an assessment & recovery approach with people accessing secondary mental health services for severe and enduring mental illness.
The post holder will be responsible for the assessment of care needs, the development of programmes of care and the implementation and evaluation of these programmes of care for a defined clinical caseload including the role of a Care Co-ordinator within the Care Treatment Planning under the Mental Health (Wales) Measure 2010.
The post holder is required to work without direct supervision for their delegated caseload. The post holder will have effective communication skills and develop positive working relationships between all healthcare professionals, statutory and voluntary agencies. The role will cover 7 days per week and will require some weekend working.

- Professional Qualification/Registration from recognised body – i.e.: RMN, Occupational Therapist or Social Worker
- Post-basic education in the area of practice to postgraduate diploma level
- Knowledge and application of care co-ordination
- Understanding of the area of current practice
- Degree in relevant professional field
Experience
- Wide range of experience within a service related setting
- Demonstrates achievements in developments of care standards/procedures guidelines
- Demonstrates effective team working
- Demonstrate ability to use computer systems competently e.g. FACE, Datix, Intranet, ESR, Internet and e-mail.
- Demonstrates research and audit experience
