Locum Reablement Occupational Therapist (Hospital-Based)
We have an exciting opportunity for a dedicated professional to join our Reablement / Hospital Social Work Team within the Integrated Discharge Hub.
Based in Livingston in Scotland
36 hours per week
Role Overview
- This role is based within a busy hospital setting, focusing primarily on reablement and assessment work.
- You will play a key role in supporting patients to regain independence and ensuring safe, effective discharge pathways back into the community.
- Working as part of the Integrated Discharge Hub, you’ll collaborate closely with multidisciplinary teams and community services to deliver high-quality, person-centred care.
We’re seeking someone with:
- A background in Occupational Therapy (OT) or reablement services
- Experience in assessment within hospital or community settings
- Strong understanding of discharge-to-assess models
- Excellent communication and teamworking skills
What You’ll Be Doing
- Supporting patients with reablement-focused interventions
- Conducting assessments to facilitate safe discharge
- Liaising with community services to ensure continuity of care
- Working collaboratively within a multidisciplinary team
