Overview
As a Clinical Lead Nurse within the Urgent Care Response (UCR) team based at Sheppey Community Hospital in Swale, you will provide clinical oversight and operational coordination for the UCR service. This full‑time leadership role (37.5 hours per week) involves delivering high‑quality, responsive care to patients in their own homes and preventing unnecessary hospital admissions.
Salary: £49,387 – £56,515 (Band 7) with NHS Pension, full Agenda for Change terms and conditions. Additional benefits include:

Job Responsibilities
- Provide clinical leadership and operational oversight for the UCR team.
- Manage day‑to‑day delivery of the service, ensuring safe, effective and patient‑centred care.
- Undertake advanced assessment, planning, implementation and evaluation of care for patients with complex needs.
- Act as an autonomous practitioner, making sound clinical decisions and delegating appropriately.
- Support, supervise and develop team members, promoting high standards of practice.
- Ensure all documentation is completed in line with NMC standards and HCRG Care Group policies.
- Monitor service performance and contribute to continuous improvement initiatives.
- Respond to urgent issues involving both patients and staff, providing effective leadership during challenging situations.
Qualifications and Requirements
- Current registration with the Nursing and Midwifery Council (NMC).
- Evidence of continuing professional development and education at degree level or equivalent.
- Leadership or management qualification, or willingness to work towards one.
- Teaching and assessing qualification or equivalent experience.
- Significant post‑registration experience, including community nursing experience.
- Experience of leading teams and managing services.
- Experience of caring for patients with complex and long‑term conditions.
- Strong clinical decision‑making and organisational skills.
- A full UK driving licence and access to a vehicle for work purposes.
