SPECIFIC RESPONSIBILITIES The post holder will have clinics of booked appointments.

They will include a combination of face to face and telephone appointments.

During these appointments they will: Undertake a comprehensive assessment of a patients mental health needs.

Assess and manage risk to patients and others.

Formulate appropriate interventions, including prescribing medication.

Update patient records contemporaneously including appropriate clinical coding.

When necessary, liaise with and refer to secondary care.

The post holder will manage the review of patients identified with Mental Health and Dementia, within the Quality and Outcomes Framework Managing all associated Reports, discharge summaries and blood results identified with Mental Health and Dementia The post holder will work collaboratively with colleagues and support them with advice and guidance as required.

Carry out planned audit of mental health prescribing and referral.

Participate fully in the effective monitoring, review and evaluation of the service provided.
