We are seeking a dedicated and compassionate Practice Nurse to join our friendly and supportive primary care team. You will actively contribute ideas, support service improvements, and help drive forward high standards within the practice. In this role, you will work closely with GPs, Practice Nurses, Nurse Associates, Healthcare Assistants and administrative staff, building strong working relationships to ensure seamless, coordinated care. Deliver high-quality nursing care within the practice. Conduct patient consultations (face-to-face and telephone). Assess, plan, implement and evaluate individual care plans. Lead and support clinics for diabetes, asthma, COPD, and cardiovascular disease. Contribute to the management of other long-term conditions. Immunisations & Vaccinations Administer childhood immunisations in line with national guidelines. Provide travel health advice and vaccinations. Deliver seasonal vaccination programmes (e.g. flu, RSV, Pneumococcal). Health Promotion & Screening Treatment Room Duties Wound care, dressings, and suture removal. Process pathology tests and results. Participate in audits and quality improvement. Engage in ongoing CPD and training. Work collaboratively with GPs and the wider healthcare team. Registered General Nurse with valid NMC registration. Diploma/BSc level qualifications in diabetes, asthma, or COPD. Immunisation and vaccination trained. Strong understanding of QOF and safeguarding. At least 2 years experience in primary care. Full UK driving licence. Opportunities for professional development and training.
