Junior Sister / Charge Nurse – PATCH

Due to maternity leave, this role offers an exciting opportunity for an enthusiastic and experienced paediatric nurse to join our dynamic, forward‑thinking paediatric hospital home team for fixed maternity cover. The position is part of the PATCH team – “Providing Assessment and Treatment for Children at Home”. This nurse‑led service delivers telephone reviews and/or home visits for children who would otherwise be admitted to hospital, with a focus on health education, illness promotion, and reducing PED re‑attendances.
Key responsibilities include:
- Conduct telephone reviews and home visits for paediatric patients
- Provide acute assessment and treatment at home
- Audit and review existing care pathways for common minor conditions
- Disseminate educational resources to professionals across the urgent care pathway for children
- Collaborate with Paediatric ED, PAU, and community teams to ensure seamless care pathways
- Support the implementation of an innovative model of nursing care and contribute to service development

Qualifications & Skills
- Nursing degree
- Valid NMC RN PIN
- Paediatric Intermediate Life Support course
- Driving licence and willingness to travel to visits independently
- Experience at band 6 in acute/general paediatrics
- European Paediatric Life Support
- A&E Clinical Care/High Dependency Course
- Mentorship or teaching qualification
- Experience in enhanced paediatric assessment
