Clinical Team Leader

Act as a senior clinical professional within the UCR service, providing an urgent rapid response to Warrington residents experiencing a health crisis. Support patients to remain at home rather than be admitted to hospital. Liaise with patients, the team and colleagues across the wider health and social care environment including NWAS. Promote, maintain and enhance effective multidisciplinary patient care.
Provide specialist treatment and advice to patients and staff, contributing to assessment, diagnosis, care and treatment at home to prevent unnecessary hospital admission. All referrals triaged immediately and responded to within two hours, prioritising referrals from NWAS.
Demonstrate extended clinical skills in assessment, diagnosis, treatment and management of a wide range of patient care needs. Exercise professional autonomy and clinical judgement in managing, referring and signposting patients onto wider community services and assets.
Be responsible for the clinical management of the UCR Team, ensuring effective and efficient delivery of a high‑quality timely service to meet commissioner requirements. Plan capacity and demand to deliver a safe service and contribute to admission avoidance and timely discharge home.
Key Duties
- Lead and manage the UCR Service, collaborating with stakeholders to ensure patients’ needs are met.
- Provide leadership and support by promoting professional development and encouraging clinical supervision.
- Proactively manage capacity to meet service demands.

- Ensure timely accurate information on annual leave, expenses and study leave.
- Monitor and report to line manager on HR, safeguarding and health and safety issues, taking action as appropriate.
- Actively manage sickness and absence of staff, accessing support from line manager as needed.
- Ensure effective use of physical and financial resources, ordering supplies and equipment.
- Distribute rotas equitably and prioritise workload.
- Monitor, report, audit and evaluate that staff maintain appropriate and accurate patient records.
- Oversee collection of monitoring data and contribute to performance management.
- Lead on the clinical aspect of complaints, taking action to ensure learning from patient/service‑user feedback.
- Investigate incidents relating to nursing practice, ensuring learning across the service.
- Assist line manager to recruit, motivate, develop, support and induct staff in line with policy and service priorities.
- Manage own time and that of the nursing team, providing rota cover as required.
- Balance responsibilities, aiming for 80% leadership/management and 20% clinical duties to maintain clinical skills.
Clinical Responsibilities
- Complete comprehensive assessment of highly complex nursing needs. Plan, assess, implement and evaluate care delivery using prescribing skills.
- Ensure nursing team practice is evidence‑based through audits and critical appraisal of literature.
- Contribute to multidisciplinary meetings and maintain effective communication with individuals, groups and communities.
- Involve patients and carers in care planning and delivery.
- Perform range of clinical skills related to the post, maintaining accurate records per NMC and Care Quality Commission standards.
- Identify training needs, undertaking PDR and demonstrating ability to work without direct supervision.
- Demonstrate problem‑solving and collaborative inclusive working in complex situations.
