Eligibility: This role is open to colleagues currently employed within Kent and Medway NHS Trusts, Kent and Medway Integrated Care Board (ICB), and Medway Community Healthcare. Nursing vacancies are also open to student nurses and newly qualified nurses.
Position Summary
We are seeking a motivated and clinically confident Community Diabetes Nurse Specialist to join our community nursing service. The role sits at the heart of our home‑visiting team, supporting housebound patients with complex diabetes needs and providing expert clinical leadership to colleagues. You will act as a senior clinician within the service, using advanced assessment skills, sound clinical judgement and specialist diabetes knowledge to guide care planning, adjust prescriptions and reduce unnecessary visits where appropriate.
Key Responsibilities
- Provide advanced clinical assessment and management for patients with complex diabetes in their own homes, ensuring safe, effective and holistic care.
- Support community nursing teams by reviewing and adjusting diabetes treatment plans, offering expert advice and helping reduce avoidable hospital admissions.
- Teach and support staff—formally and informally—across key areas such as insulin management, end‑of‑life care, IV therapy and wider community nursing competencies.
- Ensure safe staffing, oversee team performance, manage incidents and complaints, and maintain high standards of clinical governance.
- Identify quality improvement projects, present proposals to senior leaders, manage budgets, ensure supervision frameworks are in place and provide hands‑on clinical support when needed.
- Act as a visible clinical leader, supporting and developing colleagues, role‑modeling best practice and helping maintain a confident workforce.
Qualifications and Experience
- Level 6 degree in Nursing and completion of the SPDN course.
- Experience within your area of practice, managing complex caseloads and supporting/mentoring others.
- Registered with the Nursing and Midwifery Council or Health and Care Professionals Council.
- A full, valid UK driving licence or a full driving licence issued by an EU country.
- Willingness to undertake postgraduate diabetes training if not already completed.
Knowledge and Skills
- Customer‑focused approach providing patients, clients and their families with the best possible experience.
- Ability to use IT systems and databases effectively.
- Knowledge of up‑to‑date clinical practice, use of evidence‑based procedures and clinical effectiveness frameworks.
- Understanding of National Service Frameworks and NICE guidance.
- Aware of developments within the NHS.

Salary
£49,387 to £56,515 a year pro rata per annum.
