Integrated Care Clinician (Nurse). Ty Elwy, St. Asaph
The post holder will undertake telephone triage assessments of patients in an efficient way. They will advise the most appropriate intervention to meet the needs of the patient using best clinical practice, adhering to Trust Policy, protocols and guidelines.
Will work in an autonomous capacity, assessing and advising callers within agreed guidelines.

This dynamic role draws upon your clinical assessment skills to help and advise a broad spectrum of patients, from minor ailments and injuries to complex and urgent conditions and to hone your skills in a remote clinical decision‑making environment.
As a Band 6 Triage and Assessment nurse, you will have the autonomy to make decisions but be supported by our experienced team of senior clinicians, practice coaches and multi‑disciplinary colleagues.

Person Specification
- NMC registered nurse (adult or child) or HCPC registered paramedic with demonstrable post‑registration experience.
- Evidence of undertaking regular update training and continuous personal/professional development as required by the regulating body.

- Flexible approach to work – expect to work a rotating shift system including nights and weekends.
