Complex Care Liaison Nurse

To give full support with complex discharges, support ward staff with attending meetings regarding continual health care, liaise with several members of the MDT and outside parties.
Main duties of the job
To provide quality assurance on all documentation received for funded nursing care (FNC) and continual health care (CHC) e.g., fast track discharges, liaise with MDT members internally and externally and assist with arranging decision support tool meetings (DSTs).
Excellent communication skills are required and a good working knowledge of Excel.

This isn’t ‘fluency’, just speaking & listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health.
