We’re looking for experienced:
- Occupational Therapists with community mental health experience and strong assessment skills.
The role
As a Band 6 Triage Practitioner, you’ll be part of the “front door” of the Wealden & Lewes Neighbourhood Mental Health Team.
You’ll help manage referrals into the service, ensuring people are assessed, prioritised and supported appropriately based on clinical need.
Working alongside two experienced Senior Nurse Practitioners, you will:
- Screen and triage referrals into the service
- Undertake initial mental health and risk assessments
- Support care planning and safety planning
- Liaise with GPs, Primary Care and partner agencies
- Help ensure referrals are managed within agreed timescales
- Contribute to MDT discussions and service development
This is a fast-paced and rewarding role where strong clinical judgement and communication skills are essential.

The Wealden & Lewes Neighbourhood Mental Health Teams work closely with:
- Primary Care services
- Community and voluntary sector partners
- Specialist mental health services across Sussex
The aim is to provide more joined-up, responsive support within local communities.
Location
This role is based in Newhaven, a coastal town situated between Brighton and Eastbourne and surrounded by the South Downs National Park, offering a balance of seaside living and easy access across Sussex.
What we’re looking for
You’ll be someone who:
- Has experience working with people with complex mental health needs
- Is confident undertaking mental health and risk assessments
- Can prioritise and manage referrals effectively
- Communicates professionally and compassionately under pressure
- Works well within multidisciplinary and multi-agency settings
Community mental health experience is essential.
