Role Overview
We are looking for one mental health practitioner (1.8 WTE) from a professional background of nursing, occupational therapy or social work who can demonstrate excellent assessment and communication skills and deliver a compassionate, person‑centred and high‑quality service to patients, carers and others referred to the Neighbourhood Mental Health Team.
Key Responsibilities
- Assess and manage risk in mental health with patients experiencing a crisis in the community.
- Provide assessment, treatment and support to patients, carers and other stakeholders.
- Carry out home visits and see patients on site at Mill View Hospital.
- Forge links with the voluntary sector and specialist teams within the Trust, Primary Care and Partnerships.
- Engage in regular clinical supervision and pursue opportunities for training and personal development.
Qualifications & Skills
- Experienced mental health practitioner with a background in nursing, OT or social work.
- Excellent assessment and communication skills.
- Ability to deliver compassionate, person‑centred care.
- Experience assessing and managing risk in mental health.
- Motivated, enthusiastic, creative and resourceful with a passion for working with people and developing good relationships with carers.
