Associate Clinical Nurse Specialist (Palliative Care)
Location: Sydenham and BromleyDepartment: Community Palliative Care TeamHours: 37.5 hours per week (Monday–Sunday on a rota basis)
We are looking for compassionate nurses and paramedics with experience in end‑of‑life care or a desire to develop a career in palliative care to join our community palliative care team. The service operates 7‑day a week from 7:30 am to 9 pm and includes telephone and video triage, home visits across Bromley, Croydon, Lewisham, Lambeth and Southwark, and care in our outpatient department.
Responsibilities
- Provide expert symptom control and support for people in the last phase of life.
- Offer social, emotional and spiritual care to patients and their families.
- Collaborate with the interdisciplinary team, giving advice, support and education to primary health care and hospital staff.
- Facilitate telephone and video triage and home visits as part of the Single Point of Contact and local teams.
Requirements
- NMC Pin.
- Relevant experience at Band 5 (equivalent) or above in a palliative or community care setting.
- Excellent oral and written communication skills.
- Ability to work efficiently in a multidisciplinary team and manage stressful situations.
