Main duties of the job
This role requires a dedicated healthcare professional committed to upholding high standards through evidence-based practice, rigorous record-keeping, and adherence to clinical governance frameworks. Essential skills include effective, empathetic communication across diverse audiences, collaborative teamwork, and adaptability within a multidisciplinary environment. The ideal candidate will demonstrate professionalism, cultural sensitivity, and proactive engagement with continuous learning and development, ensuring quality patient care at all times.
A good understanding of respiratory care and current evidence based best practice guidelines is essential.
The successful candidate will be required to manage their own caseload within the team, with support from the senior respiratory nurse.
- The post-holder will support the Team Leader in the delivery of the Breathe Service in community.
- To manage a defined clinical caseload, providing specialist support, education and complex case management to people with respiratory conditions and their families and carers.
- Work as part of a wider Neighbourhood Multi-disciplinary team of nursing, therapy, mental health, social care and primary care staff.
- Liaise with primary and secondary care colleagues, providing education and support, thus developing and maintaining high quality evidence based respiratory care across Barnsley.
Job responsibilities
JOB SUMMARY
- Support the Team Leader in delivering the Breathe Service in community settings.
- Manage a defined clinical caseload, offering specialist support, education, and complex case management.
- Collaborate with multidisciplinary teams and coordinate care across primary, secondary, and community services.
- Provide high quality, evidence‑based respiratory care and maintain professional competence.
KEY RESULT AREAS:
- Work autonomously assessing and managing own clinical caseload for all clients/patients within a designated neighbourhood.
- Apply specialised skills underpinned by advanced theoretical knowledge to maintain professional competence and fitness to practice.
- Demonstrate evidence based knowledge, investigative and analytical skills and specialised assessment techniques to triage, assess and manage a range of complex presentations.
- Formulate and implement treatment plans based on current evidence and expert clinical reasoning, integrating self‑management principles.
- Enable early and accurate diagnosis and support to manage respiratory disease.
- Participate in multi‑disciplinary team meetings, contributing to patient‑care decisions.
- Work alongside Primary Care GPs and Practice Nurses to manage patients within the Primary Care Network.
- Collaborate with respiratory and medical teams within secondary care to facilitate early supported discharge.
- Manage referrals to the service and ensure timely assessment and support for patients in crisis and end‑of‑life care.
- Initiate, plan and implement induction, training and education for junior team members, community nursing teams and students.
- Provide supervision and caseload support to junior team members.
- Understand National Standards (NICE, NSSF, BTS) and apply them to service delivery.
- Lead development and delivery of specialist education programmes for professionals.
- Contribute to policy, procedure and standard development using an evidence based approach.
- Support clinical governance framework to maintain high standards of care.
- Undertake Nurse Prescribing in accordance with trust policy.
- Maintain complete, accurate and up‑to‑date records in line with NMC guidelines and trust policy.
- Adhere to NMC scope of practice and trust policies, ensuring professional competency through mandatory training.
Communication with others:
- Communicate effectively, empathically and in a culturally sensitive manner with patients, representatives, carers and family.
- Communicate with all members of the Breathe Service to provide cohesive service.
- Work with other health professionals, statutory and voluntary agencies to ensure a team approach.
- Communicate with SWYPFT GP practices and health professionals in other areas.
Person Specification
- Artp accredited or willingness to undergo accreditation upon commencement in post.
Training
- Ability to undertake training in advanced respiratory nursing and community care practices.

- Recent knowledge of COPD, Oxygen and Respiratory care including acute exacerbation management.
- Excellent communication and interpersonal skills.

- Sound understanding of clinical governance and audit.
- Ability to network and form professional relationships with healthcare and multi‑agency professionals.
- Sound IT skills (Microsoft Word, PowerPoint, Outlook and internet).
Experience
- Recent experience of respiratory care and clinical examination.
- Substantial experience of working as part of a nursing and multidisciplinary team.
- Recent experience of working in the community setting.
Physical Attributes
- Ability to undertake duties and demands of the post. Satisfactory sickness record over the previous 2 years.
Qualifications
- Graduate level education and/or working towards Masters level.
- Diploma/Degree in Respiratory Disease or similar.
- Professional teaching qualification.
