This is an exciting time to work in Cambridgeshire South as we accelerate our Neighbourhood Health Programme – an ambitious, system-wide effort to redesign how care and support is delivered across our Place.
The Integrated Neighbourhoods Project Manager will provide transformation support and project management to help develop Integrated Neighbourhoods and build relationships between organisations.
We are looking for a Neighbourhood Project Manager to join our small innovative team for a up to a 12 – month period; offered as a secondment or fixed term contract. This opportunity would be perfect for anyone keen to learn more about and support the development of Neighbourhoods across Cambridgeshire South area and specifically in the East Cambridgeshire locality.
Your role will be to work collaboratively with partners and Communities to align services and support, continue to embed our relational way of working and support the ongoing development of our Neighbourhood Teams, to recognise opportunities and identify how they can improve care and support and then help to deliver this change.
You will act as a relationship builder, a connector, a problem solver and a cheer leader, always promoting the neighbourhood ways of working at a time of significant pressures and challenges. You will provide transformation and project management expertise to make positive changes, build strong and sustainable foundations for Neighbourhood Teams and to tackle inequalities.
Main duties of the job
- Provide transformation support and project management for the Integrated Neighbourhoods programme ofwork.
- Review population health data and support public engagement to determine local needs and how citizens can shape the improvement of their health and care support.
- Be responsible for supporting the service redesign and delivery of specific projects within the Integrated Neighbourhood particularly around long-term conditions and health inequalities.
- Work closely to build relationships and bring partners and communities together to create a shared vision, priorities and make positive changes to the way we collaboratively support our community’s health & well-being.
- Work closely with Clinical leaders, Primary Care, the acute sector, community services, health commissioners, local leaders, local authorities, voluntary and community organisations, and the public to support the development of the service design.
- You may line manage project support officers and will work closely and collaboratively with the other Integrated Neighbourhood Project and Programme Managers.

Detailed job description and main responsibilities
The Integrated Neighbourhood programme aims to co-develop a Neighbourhood Health Service that supports people to stay well in their own communities. Working in partnership with Primary Care, Community Services, local authorities, the voluntary sector, we are developing new models of support and care to deliver against the national Neighbourhood Health Framework.

We are particularly interested in anyone with experience in data collection, analysis and evaluation – measuring what matters most to people, communities and our Neighbourhood workforce , and co-designing services around this , is a key focus of the role in East Cambridgeshire. We are looking for someone who enjoys working with people and bringing the best out of them, a person with confidence to work autonomously and opportunistically in a constantly evolving environment.
As this is a short-term position for up to 37.5 hours per week, secondments (please seek your managers permission before applying) and flexible working / reduced hours proposals are welcome.

- Educated to degree level with further training to post graduate diploma level or equivalent professional experience

- Evidence of successful delivery of an organisation wide project in a service improvement role
- Experience of planning organisational resources to support programme / project implementation
