About The Organisation

The Role

  • The Advanced Practitioners will work autonomously across Primary, Community and Acute Services, covering in‑and out‑of‑hours periods and seven days including public holidays. They will be responsible for clinical decision‑making for the population, including assessment and treatment of patients across these settings.
  • The Advanced Practitioner will work closely with the multidisciplinary team (MDT)/medical staff, including the OOH Hub and Acute Services, communicating daily about any patient seen overnight or on the weekend who needs follow‑up by the GP or Acute Services during normal working hours.
  • The Advanced Practitioner provides assessment, clinical care and management of both stable and acutely unwell patients and their families, making complex autonomous decisions using advanced skills and knowledge. The practitioner will have overall responsibility for assessing care needs and delivering advanced care, with medical telehealth/telephone support available when required from an identified source.

In This Key Role, You Will

  • Autonomously assess, diagnose and treat individuals with a range of acute, non‑acute and chronic presenting medical conditions across the lifespan.
  • Utilise advanced knowledge and highly specialist clinical skills in a range of settings to assess, plan, implement and evaluate programmes of care, delivering the highest possible standard of care to patients and their families within available resources and referring to appropriate agencies.
  • Practice at all times within the Professional Codes of Conduct.
  • Conduct evidence‑based practice in accordance with clinical guidelines and standards.
  • In partnership with the multidisciplinary team, develop, implement and maintain policies, procedures, standards and protocols of Advanced Practice.

What You’ll Bring

  • Registered Nurse / AHP / Pharmacist with valid NMC/HCPC/GPHC Registration.
  • Educated to/working towards/operating at Masters Level.
  • Post Graduate Diploma in Advanced clinical practice and expertise within speciality /
  • area of practice that demonstrates the required breadth of knowledge to lead safety, effectively and efficiently.
  • Comprehensive knowledge of clinical guidelines and standards within the speciality.
  • Advanced Specialist knowledge and experience of relevant conditions, pathology, policies and procedures associated with the speciality/area of practice ensuring that the level of expertise can be utilised to deliver leadership within speciality.
  • A driving licence is required; this must be a full UK/EU/EEA licence.
