Community Support Worker – Team Leader

Overview

We are a highly regarded not-for-profit, values-based organisation, offering residential, community and in-home aged care for over 30 years, to older people living in and around the city of Manningham.

• Part Time (19 hours per week)

Responsibilities

• Provide leadership, guidance and support to a team of Community Support Workers
• Mentor and coach staff to support performance and development
• Ensure services are delivered in line with care plans and client goals
• Occasionally provide direct client support including personal care and activities
• Ensure services comply with Home Care Standards and MannaCare policies
• Monitor service quality and ensure accurate completion of case notes and incident reports
• Escalate staff or service delivery issues to management when required
• Work in accordance with Occupational Health and Safety requirements and complete mandatory training

About you

• Certificate III in Individual Support or Certificate IV in Ageing Support
• Minimum 5 years’ experience as a Community Support Worker or similar role
• Demonstrated experience working in aged care or community services
• Strong leadership, communication and interpersonal skills
• Ability to write progress notes and follow care plans
• NDIS Worker Screening Check
• Current Victorian driver’s licence and access to a reliable vehicle if required
• Basic computer literacy and ability to use client management systems
