Main duties of the job
The post holder will work as part of a multi‑disciplinary team and provide clinical pharmacy leadership for the practice, taking responsibility for structured medication reviews, chronic disease management, medicines optimisation, quality improvement and the quality and outcomes framework. The holder will proactively manage complex polypharmacy patients, support GPs in implementing effective medication management, and monitor budgets.
Job responsibilities
Manage repeat prescribing systems, handle acute prescription requests, perform medicine reconciliation on transfer of care and put in place safer prescribing systems. Provide clinical medicines advice, support practice staff with prescription and medication queries, and integrate with community and hospital pharmacy to improve patient outcomes and reduce GP workload.
Lead and maintain quality improvement initiatives, assist in searching and auditing for improvement opportunities, and implement change management to enhance operational efficiencies.
Person Specification
Qualifications
- Full‑time working as a pharmacist in general practice for more than 2 years as a Practice‑Based Pharmacist.
- Clinical Pharmacy Diploma or equivalent training such as NHS Pathway training for PBP’s Independent Prescriber.
- Experience with teaching, training, clinical supervision, mentoring, and coaching for change.
Experience
- Management of self and others to achieve goals and targets.
- Experience working as a clinical pharmacist in general practice.
- Proficiency in medication reviews, chronic disease management, and medicines optimisation.
