Community Transfer of Care Hub Administrator (North Somerset)
Band 3
Contract: Permanent, Grade Band 3.
Hours: 8:00 – 18:00, 7 days a week including bank holidays.
Working scheme: Part-time, flexible work 34.5 hours per week.
Job Overview
Enthusiastic administrator to support the clinical and admin teams within the Community Transfer of Care Hub (CToCH). The role focuses on data collection to evaluate service impact and collaborates with acute hospitals, social care, primary care, and the Integrated Care Board.
Main Duties
- Provide comprehensive admin support to health and social care professionals, hospitals, GPs, service-users, relatives and carers.
- Answer telephone enquiries in a friendly and professional manner.
- Maintain accurate and up-to-date patient records and data in Sirona IT systems, electronic patient records, databases and spreadsheets.
- Assist in data collection and reporting to evidence the impact of new ways of working on flow, bed days saved and patient experience.
- Support the delivery of the admin component of the CToCH.
Qualifications
- Functional literacy and numeracy level 3/GCSE grade A-C or equivalent, including English Language and Mathematics.
- NVQ Level 3 Business and Administration or equivalent experience.
Skills, Knowledge and Experience
- Ability to take responsibility for tasks and work autonomously.
- Time‑management skills and ability to work under pressure.
- Experience in a demanding office environment and meeting deadlines.
- Proficiency with IT systems, including Clinical Systems and Microsoft Office.
- Competence in handling sensitive and confidential issues with discretion.
- Previous acute or community care experience.
- Workload prioritisation capabilities.
- Experience with cloud‑based telephony systems.
- Use of EMIS.
