The post holder is anexperienced qualified pharmacist acting within their professional boundaries.
The post holder will workwithin a multi disciplined team to provide support to general practice staffwith regards to prescription and medication queries, support the repeatprescriptions system, deal with acute prescription requests, medicinesreconciliation on transfer of care and systems for safer prescribing, providingexpertise in clinical medicines advice while addressing both public and socialcare needs of patients in the GP practice/s.
The post holder willprovide clinical leadership in the GP Practice on medicines optimisation andquality improvement and manage some aspects of the quality and outcomesframework and enhanced services. The postholder will be working towards orutilising their non-medical prescribing qualification.
The post holder will ensure that thepractice integrates with community and hospital pharmacy to help utilise skillmix, improve patient outcomes, ensure better access to healthcare and helpmanage workload. The role is pivotal to improving the quality of care andoperational efficiencies and therefore requires motivation and passion todeliver excellent service within general practice.

3 Valleys Health PrimaryCare Network is a group of 8 general practices covering circa 68k patientsliving in rural and semi-rural North-East Somerset and within easy commutingdistance of Bath, Bristol and Wells.
Currently we have a well-establishedand experienced team of 10 Clinical Pharmacists and 7 Pharmacy Techniciansworking within the PCN. Each colleagueis embedded in one or two practices only and at the same time they are part ofa bigger PCN supportive peer group which meets regularly. There areopportunities to develop areas of special interest and participate in PCNprojects within the role.
We would like to expand ourPharmacy team further and have a vacancy in the Midsomer Norton/Radstock area.
Successful candidates willbe enrolled on the PCPEP Primary Care Education Pathway and subsequently anIndependent Prescribing course (if not already qualified) and be mentored by aGP.

Experience
- Indepth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare
- An appreciaton of the nature of GPs and general practice
- An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
- Excellent interpersonal, influencing and negotiating skills
- Excellent written and verbal communication skills
- Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (eg patients)
- Able to plan, manage, monitor , advise and review general medicine optimisation issues in core areas for long term conditions
- Able to obtain and analyse complex technical information
- Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate

Qualifications
- Registration with the General Pharmaceutical Council (GPhC)
- Independent Prescriber or working towards/intent of gaining an independent prescribing qualification
- Membership of the Royal Pharmaceutical Society
- A member of or working towards Faculty membership of the Royal Pharmaceutical Society
- Specialist knowledge acquired through post-graduate diploma level or equivalent training or experience
- Completed or working towards a Primary Care Pharmacy Education pathway qualification
