Position Overview
This is an exciting opportunity for a qualified healthcare professional with significant experience of working within mental health services to join a friendly, supportive and diverse team. This is an innovative new post, integral to the operational delivery of our Inpatient Flow Quality Improvement Programme. The successful applicant will join a supportive multi‑disciplinary team, and will be responsible for facilitating transfers of care, ensuring patients are receiving care congruent with their needs. The post holder will manage a case load of patients experiencing a delayed transfer of care, lead the coordination and progression of the patient’s transfer by engaging with internal clinical teams and external agencies to overcome barriers to discharge. The candidate should possess a sound understanding of forensic discharge protocols and procedures, experience resolving funding issues that arise during the discharge process, and be proactive with excellent communication skills to work collaboratively with stakeholders. They should also have experience of designing and delivering quality improvement projects.
Main Duties
- Proactively address delayed transfers of care across the service.
- Provide transfers expertise and knowledge as appropriate, regarding pathway issues internally and externally, including developing and delivering training to share knowledge.
- Achieve and report key performance measures, such as achieving a 10% decrease in delayed discharge days.
- Proactively identify and challenge barriers to care progression.
- Timely monitor failed transfer of care plans, leading and planning reviews and devising alternative strategies.
- Identify patients who are approaching discharge and support ward teams to overcome associated barriers, which may involve attending some ward rounds.
- Attend relevant meetings, including Weekly Referrals and CRFD Meeting, and Monthly Pathway Management Meeting.
- Act as a point of contact between internal teams (e.g. SCFT and inpatient services) and external agencies.
- Act as a point of contact for our patients, keeping them updated on their transfer of care or discharge progress.
Qualifications
Essential
- Significant experience of working in mental health services.
- Experience of working with senior stakeholders across health and social care.
- Experience of the complexity of funding issues, blockages and challenges relating to discharge and evidence of innovative practice to overcome these.
- Knowledge of forensic discharge protocols and procedures.
- Experience of working in forensic mental health services or community mental health setting.
- Professional registration with relevant healthcare body.
- Evidence of continuous professional development.
- Understanding of NHS legislation and national strategy including the Care Act, Health and Care Act, MHS, MCA, Who Pays Guidance, etc.
- Well‑developed planning and organisational skills, ability to work under pressure and meet deadlines.
- Proven ability to build relationships at all levels and across multiple disciplines.
Desirable
- Educated to master’s level or equivalent level of experience of working at a senior level.
- Leadership or management qualification.
