An excitingopportunity has arisen for an enthusiastic motivated pharmacist to join our innovative,award-winning pharmacy team. Based acrossSouth Kerrier PCN, our patient population of just over 33, 000 patients spreadacross five practices: Helston Medical Centre, Mullion & Constantine Group,Cober Valley Health, St Keverne Health Centre and Isles of Scilly St MarysHealth Centre. Our pharmacy team arehighly valued, well integrated and respected amongst our PCN and individualpractices.
The team iswell established with a senior lead clinical pharmacist, a senior audit/systemspharmacist, clinical pharmacist & three pharmacy technicians . With a strong trainingethos, as a team we are involved in training both undergraduate and graduatepharmacy staff as well as the wider MDT. We have a rolling audit programmewhich has been acknowledged at local and national level and are well recognisedfor our success in improving patient safety.We are a friendly, supportive, and welcoming team and very much value ourmembers and wholly support their development. We hold regular team meetings,monthly peer review and supervised and mentored sessions.
Working inGeneral Practice no two days are the same. The role is dynamic and exciting. Weoffer training and the opportunity to develop in areas ofinterest providing asupportive learning environment. We offer ongoingsenior review, and technician support and the opportunity for individualised,structured project work.
Main duties of the job
You will beworking as part of a wider practice and PCN team including GPs, nurse andparamedic prescribers, social prescribers, and physios. You will providesupport to practice staff, both clinical and non-clinical, responding toqueries, completion of medicines reconciliation and short- and long-termcondition management. You will have a regular clinical commitment completing virtual,telephone and face to face reviews. You will be expected to support the teams medicinessafety programme and dedication to improving overall patient care and safety,whilst observing the local and national medicines optimisation agenda.

Job responsibilities
Thepost holder is a pharmacist, who acts within their professional boundaries,supporting and working alongside a multidisciplinary team in the PCN. In thisrole they will be supported by a senior clinical pharmacist who will support,develop, manage, and mentor them.
Thepost holder will work as part of a multi-disciplinary team in a patient-facingrole. The post holder will take responsibility for areas of chronic diseasemanagement within the practice and undertake clinical medication reviews toproactively manage patients with complex polypharmacy.
Thepost holder will provide primary support to general practice staff with regardsto prescription and medication queries. They will help support the repeatprescription system, deal with acute prescription requests, and medicinesreconciliation on transfer of care and systems for safer prescribing, providingexpertise in clinical medicines advice while addressing both public and socialcare needs of patient in the GP practice (s).
Thepost holder will provide clinical leadership on medicines optimisation andquality improvement and manage some aspects of the quality and outcomesframework and enhanced services.
Thepost holder will ensure that the practice integrates with community andhospital pharmacy to help utilise skill mix, improve patient outcomes, ensurebetter access to healthcare, and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requiresmotivation and passion to deliver excellent service within general practice.
Thecandidate should have completed the 18month CPPE training programme ClinicalPharmacist in GP or be willing to undertake the training programme.
Thepost holder if not already a nonmedical prescriber will be supported to developtheir role to become a nonmedical prescriber and development process once theyqualify.
Person Specification
Qualifications
- Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
- May hold or be working towards an independent prescribing qualification
- Ideally a minimum of 2 years experience as a pharmacist, demonstrated within a practice portfolio.
Leadership
- Demonstrate understanding of the pharmacy role in governance and is able to implement this appropriately within the workplace.
- Demonstrate understanding of, and contributes to, the workplace vision
- Engages with Patient Participation Groups (PPGs) and involves PPGs in development of the role and practices
- Demonstrates ability to improve quality within limitations of service
- Reviews yearly progress and develops clear plans to achieve results within priorities set by others.
- Demonstrate ability to motivate self to achieve goals
- Promotes diversity and equality in people management techniques and leads by example.
Education, Training and Development
- Understands and demonstrates the characteristics of a role model to members in the team and/or service
- Demonstrates understanding of the mentorship process
- Demonstrates ability to conduct teaching and assessment effectively according to a learning plan with supervision from more experience colleague
- Demonstrates self-development through continuous professional development activity; working alongside senior clinical pharmacist to identifying areas to develop
- Participates in the delivery of formal education programmes
- Demonstrates an understanding of current educational policies relevant to working areas of practice and keeps up to date with relevant clinical practice.
- Ensures appropriate clinical supervision is in place to support development
- Enrolled into review and appraisal systems within the practice
Experience
- Ideally have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice
- Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate
- Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct
- Involves patients in decisions about prescribed medicines and supporting adherence as per NICE guidelines.
