Admission Avoidance Sister/Charge Nurse/HCP
Band 6 – Part time 15 hours. Permanent contract.
Job Summary
This is an exciting opportunity for a skilled nurse or health care professional to join the Admission Avoidance Team (AA). As a member of the AA Team, you will work as a senior clinician with a broad range of competencies, adaptable to a wide variety of clinical circumstances. The team is based on the Acute hospital site and focuses on conducting holistic assessments across the Emergency Department, Acute Medical Unit, Integrated Assessment Unit, Same Day Emergency Care and Urgent Treatment Centre to support medically optimised patients for discharge using pathways such as home first, discharge to assess and Stamford unit referrals.
Main Responsibilities
- Conduct holistic assessments to support discharges and avoid admission, using the home first and discharge to assess pathways.
- Work closely with health and social care colleagues across acute and community settings to support safe discharges.
- Refer patients onto appropriate services to prevent readmission.
- Attend unit board rounds daily to identify patients who can be supported home with Admission Avoidance to facilitate earlier discharge.
- Provide complex assessments and interventions, making high‑impact clinical decisions to support patients remaining in their own homes.
- Work autonomously while drawing assistance from primary and secondary care colleagues.
- Use evidence‑based principles, current best practice and specialist skills to assess, plan, implement and evaluate interventions.
- Manage caseload and communicate to prioritise patient care; collaborate with senior clinicians, practitioners, social workers and other professionals to establish a whole‑systems approach.
- Complete rapid discharges for palliative and end‑of‑life patients to their preferred place of care and conduct assessments to support placement into appropriate facilities.
