An exciting opportunity for a experienced and highlymotivated registered nurse to join our Community Frailty Hub team.

The successful post holder will work as a dynamic and proactive practitioner supervising a team of nurses and health supportworkers with the support from the clinical lead within the Integrated Neighbourhood Teams (INT) of the City. You will be based in the community, working collaboratively as part of our Frailty Hub MDT supporting the team and conducting proactive nurse led Comprehensive Geriatric Assessments to people living with Frailty in York, to support them to live well for longer in their own homes.

You will also work as part of the Complex Frailty INT in which we work closely with patients with complex needs. The service brings together Primary Care, specialist frailty clinicians, and wider health and wellbeing partners to deliver a coordinated 9‑month intervention, focused on stabilising health, reducing risk of crisis, and supporting people to live well in their own homes.

Post 1: 37.5 hours per week

Post 2: 22.5 hours per week

Main duties of the job

Support and lead nurses and healthcare support workers to deliver high‑quality, compassionate frailty care within a multidisciplinary team.

  • Coordinate personalised patient care, ensuring accurate assessments, safeguarding, and evidence‑based interventions.
  • Promote quality, safety, and continuous improvement through audits, incident learning, and adherence to governance standards.
  • Contribute to staff development, team wellbeing, and collaborative working with healthcare and community partners.

This post requires a UK driving licence and car and includes evenings and weekend on‑call on a rota basis.

Please note that weekend on‑call duties include supporting safe and timely discharges home from hospital for patients who require a bridging package of care to prevent deconditioning and reduce the risk of hospital acquired infections.

Qualifications

  • Registered Nurse (RN) with current NMC registration.
  • Teaching, assessing or mentoring qualification.

Skills and Abilities

  • Strong coordination and supervisory skills.
  • Excellent clinical assessment and decision‑making skills.
  • Ability to work effectively as part of a multidisciplinary team.
  • Excellent communication skills, both verbal and written.
  • Strong organisational and time‑management skills.
  • IT skills, confident in Microsoft Applications.
  • Compassionate, empathetic, and patient‑centred approach to care.
  • Proactive in problem‑solving and continuous improvement.
  • Ability to manage complex patient cases with professionalism and confidence.
  • Ability to work under pressure and adapt to changing situations.

Experience

  • Previous experience in a senior role or as a nurse team lead.
  • Leadership or management qualification or equivalent experience.
  • Experience working with frailty patients or in a multidisciplinary team is highly desirable.

£38,880.72 to £41,033.12 a year (Pro Rata if part time working, salary is dependant on experience).
