Job Title: Primary Care – Practice Based Clinical Pharmacist

Annual Salary: Competitive – based on experience

Hours: Full Time/Part Time – up to 37.5 hours per week

Accountable To: Clinical Pharmacist and/or GP Clinical Supervisor

Location: Midsomer Norton/Radstock for 3 Valleys Health Primary Care Network

Overview

3 Valleys Health Primary Care Network is a group of 8 general practices covering circa 68k patients in rural and semi‑rural North‑East Somerset within easy commuting distance of Bath, Bristol and Wells.

We have a team of 10 Clinical Pharmacists and 7 Pharmacy Technicians embedded in practices and in a supportive peer group. We are expanding the Pharmacy team and have a vacancy in the Midsomer Norton/Radstock area.

Job Summary

The post holder is an experienced qualified pharmacist acting within their professional boundaries. Working within a multi‑disciplined team, they provide support for prescription and medication queries, support the repeat prescriptions system, manage acute prescription requests, conduct medicines reconciliation on transfer of care, and deliver clinical medicines advice to meet public and social care needs of patients in GP practices.

They will provide clinical leadership in medicines optimisation and quality improvement, manage aspects of the quality and outcomes framework and enhanced services, and will work towards or utilize a non‑medical prescribing qualification.

The role collaborates with community and hospital pharmacy, promotes skill‑mix utilisation, improves patient outcomes, ensures better healthcare access, and helps manage workload. This pivotal role requires motivation, passion, and commitment to excellent service within general practice.

Qualifications & Requirements

Successful candidates will be enrolled onto the PCPEP Primary Care Education Pathway and, if not already qualified, will complete an Independent Prescribing course and be mentored by a GP.

The post holder should be an experienced qualified pharmacist.
