Family Practitioner

Salary: £34,424 progressing to £37,280 per annum.

Hours: 37 hrs per week.

Location: Apsley or Stevenage offices with countywide travel required.

Contract: Permanent.

Role Overview

Provide family‑focused practice for disabled children and young people (ages 0‑25) in partnership with families, education and health professionals. Work within the 0‑25 Together Service, supporting service users in the Assessment Team, Locality Teams or Short Breaks Review Team.

Responsibilities

  • Autonomously manage a caseload of children and families, delivering Family Help and supporting social work professionals.
  • Record and present information effectively through discussion and in written reports.
  • Ensure disabled children are included and achieving their full potential, including safeguarding activities when required.
  • Provide regular, skilled professional supervision in oversight by a social work professional.

Essential Qualifications & Experience

  • Experience working directly with disabled children and families.
  • Knowledge of the social care role with disabled children and families.
  • Understanding of policy relating to children and young people with SEND.
  • Strong interviewing, recording and reporting skills.

Desirable Qualifications & Experience

  • Ability to work with complex situations and conflict.
  • Experience assessing and recording eligible and non‑eligible needs using evidence‑based practice.
  • Knowledge of working with disabled children and young people across developmental stages.
  • Experience in multi‑agency collaboration.

Additional Requirements

  • Disclosure & Barring Service (DBS) check required.
  • Driving required – ability to travel around Hertfordshire.
  • Fluency in spoken English (or provision of a sign language interpreter for application).
