An opportunity has arisen for a Community Mental Health Practitioner looking to join a great team, providing specialist mental health care to the population of Stockport. The Secondary Care CMHT supports residents with severe and enduring mental health conditions. The CMHT works with people aged 16 and over with severe and enduring mental health and social care needs.

This is an exciting time for Stockport CMHT. The service is undergoing a transformation as part of the National Transformations of community services and the local Living Well developments. It is an opportunity to be part of our service development, with planned investment in staff and new role development.

Responsibilities

  • Provide holistic assessment, interventions, risk assessments and care plans for people on your caseload.
  • Administer depot medication for people who are unable to attend the depot clinic.
  • Act as the community Mental Health Practitioner and key worker responsible for the care of an identified group of clients, supporting a recovery-oriented and socially inclusive approach.
  • Provide supervision to an identified group of staff and students on placement with the team.
