Position Details
This is a temporary role for 2–3 months, working Monday to Friday, 09:00–17:00 (full‑time), with the potential for extension depending on service need.
Role Overview
The post holder will play a key role in coordinating PCN / Neighbourhood activity, focusing on supporting the Diabetes PCN Multidisciplinary Team (MDT). You will work closely with GP practices, neighbourhood teams, and the Community Diabetes Specialist Service to support integrated, patient‑centred care delivery.
Key Responsibilities
- Coordinate the day‑to‑day delivery of the Diabetes PCN MDT / Neighbourhood services
- Support planning and implementation of new services in line with MDT priorities
- Ensure effective communication and coordination between GP practices and partner organisations
- Manage appointments, referrals, and service activity using electronic systems
- Organise and support MDT meetings and service‑related discussions
- Monitor service performance, contribute to data collection, reporting, and continuous improvement
- Track progress against objectives and support reporting of outcomes
- Work collaboratively with GPs, practice staff, multidisciplinary teams, and external partners
- Promote strong integrated working across the PCN to support efficient service delivery
