The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside GPs, nurses and the admin team in general practice. In this role they will be supported by a GP partner who will develop, manage and mentor them and be part of a network wide pharmacist forum.
The post holder will work as part of a multi-disciplinary team in a patient-facing role dealing with patient facing medication queries. The postholder will take responsibility for areas of long term chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.

Clinical Pharmacist (ARRS Funded) Part Time / Maternity Cover
We are seeking an enthusiastic and motivated Clinical Pharmacist to join our multidisciplinary primary care team at The Hollies Medical Centre. The successful candidate will work alongside GPs, nurses and administrative staff to deliver high-quality patient‑centred care, with a focus on medicines optimisation, chronic disease management and safe prescribing.
Key responsibilities include undertaking clinical medication reviews, managing complex polypharmacy, supporting repeat prescribing systems, responding to medication queries, medicines reconciliation following hospital discharge, and supporting care home medication reviews. The role also involves contributing to prescribing audits, quality improvement initiatives, QOF and enhanced services, and promoting safe and cost‑effective prescribing in line with NICE and local guidance.
Applicants should be a qualified pharmacist registered with the GPhC, ideally with independent prescribing qualifications and experience working in primary care or general practice. Excellent communication, clinical decision‑making and team‑working skills are essential.

Qualifications
