Job Description:

We are looking for a Safeguarding & QOF (Quality & Outcomes Framework) Co-ordinator on a permanent basis for 15 hours per week to join our existing friendly team. The Practice is open from 7:45am – 6:30pm each weekday, so the core hours of work will be between these hours. Experience within a customer care environment, ideally within a medical setting would be preferred. However, if you have energy, enthusiasm, a logical mind, and are self motivated with excellent communication skills and attention to detail, we would love to hear from you.

Main duties of the job:

Collaborate with healthcare professionals to prepare safeguarding reports and to ensure compliance with safeguarding policies.

Organise and minute safeguarding meetings.

Maintain the Practice Safeguarding register for both adults and children liaising with the Safeguarding leads of the Practice.

Liaise with various agencies such as local authorities, police and social services to address safeguarding concerns.

Manage Safeguarding incidents and ensure timely responses to reports of abuse.

  • Develop a safeguarding hub for resources and training to support referrals and ensure staff are up to date with safeguarding training.
  • Monitor safeguarding practices and provide updates to relevant stake holders.
  • Work with Safeguarding GP leads to ensure that Safeguarding policies are up to date and fit for purpose.
  • Recalling and inviting patients with long-term health conditions for annual reviews, facilitating access to health and social care services.
  • Collating patient data and ensuring accurate record-keeping, which is crucial for improving healthcare outcomes.
  • Implementing safety-netting processes to follow up on patients effectively, ensuring they receive appropriate care.
  • Collaborating with clinicians within the Practice to improve patient outcomes and ensure compliance with quality standards.
  • Create, amend and use searches within the clinical system to identify cohorts of patients.
  • Produce protocols and spreadsheets to provide audit.

  • We are a Training Practice and regularly host Registrars.
  • We also host year 1, 3, & 5, Medical Students.
  • We have PCN Pharmacist support.
  • We have a high QOF achievement and prevalence.

Job Summary:

The post holder will play a pivotal role in providing support and high-quality care to our patients through liaison with our Practice clinicians.

Knowledge and Skills

  • Clear, polite telephone manner.
  • Problem solving & analytical skills.
  • Competent in the use of Office and Outlook.
  • Effective time management.
  • Ability to work as part of a team, but just as capable working alone.
  • Good interpersonal skills.
  • Ability to follow Policy and Procedure.
  • Knowledge of QOF.
  • Knowledge of Safeguarding in General Practice.

Experience

  • Experience of working with the general public.
  • Maintain confidentiality at all times.
  • Experience working in General Practice.

Qualifications

  • GCSE English & Mathematics (C or above) and at least three others.
  • or
  • NVQ level 2 in Health and Social Care.
  • Educated to A-level / equivalent or higher.
