Job Summary
The Mental Health Practitioner (MHP) facilitates early mental health access and support via face‑to‑face and telephone appointments at GP surgeries.
Key Responsibilities
- Facilitate early mental health access and support through face‑to‑face and telephone appointments.
- Work alongside communities, local authorities, patients and their families to address housing, debt, employment, addictions and other social factors alongside mental health problems.
- Collaborate with care navigators, peer support workers and other professionals to combine existing services and deliver a one‑team approach.
- Provide care from GP surgeries and help shape the future of local service provision.

Location: Emersons Green Medical Centre and Leap Valley Surgery. Working days: Monday to Wednesday, subject to change.
