This post is for a 0.5 WTE Consultant Psychiatrist working within the South Islington Core Community Mental Health Team as work progresses with the transformation of community mental health services in line with the NHS Long Term plan. The post is a fixed term post and has been created to cover another consultant’s career break. The post-holder will be part of a multidisciplinary team which includes both statutory and voluntary sector roles and provides holistic care to patients in a primary care setting aligned to the North Primary Care Networks in Islington.
The role of Consultant Psychiatrist in the Mental Health Core Team offers considerable scope to work creatively and with flexibility. The multi-disciplinary team has introduced new posts such as Peer Coaches, and voluntary sector colleagues. This expansion of the social dimension of care supports people to engage in meaningful activities and links to community-based groups. All team members are encouraged to take a holistic approach, working with patients to bring meaning to their struggles and life experiences, bearing in mind the impact of trauma and social precariousness.
The Consultant Psychiatrist works closely with GPs, the MDT, and intersecting systems such as housing, social services, and the Trust Intensive Teams. The focus is on relationship‑building, reflective practice, and collaborative working.

Key Responsibilities
- Provide advice and support to Central Islington GPs and other practice staff regarding the assessment and management of mental health difficulties of patients registered with their practices.
- Provide consultant input and clinical leadership to the Central Islington Core Team.
- Assist with diagnosis, formulation, medication reviews, Dialog+ care planning, management plans, risk assessments, safety plans, and self‑help plans for patients referred to the team.
- Provide direct clinical assessments of patients registered with Central Islington GP practices and provide follow‑up as appropriate.
- Assist in the prioritisation (triage) of new referrals to the service (approximately 120 referrals per month).
- Maintain effective communication with GPs, primary care staff, patients, and their carers.
- Work flexibly within timetabled working hours to prioritise urgent clinical matters.
- Carry out Mental Health Act and domiciliary assessments as required.
- Play a key role in implementing improved linkage between primary and secondary care services.
- Liaise and work closely with local NHS Talking Therapies and other primary care providers to provide a joined‑up primary care mental health service.
- Liaise and work closely with local voluntary services as part of the new model of core community teams.
- Provide clinical input to multidisciplinary case conferencing for difficult patients managed solely in primary care.
- Engage with local service‑user participation models.
- Participate in management activities and governance processes related to the Core Teams, including audits and quality improvement projects.
- Play an integral role in the performance of the team, the further development of the service, and the development of clinical pathways and priorities of the service line.
